MOAA TAPS Form

Please use the following web form to notify MOAA of a member’s death. If the member was affiliated with a local Chapter please report the passing to them as well. A list of our Chapters is located here. We appreciate your help in keeping our member records up to date.


If you are the surviving spouse of the member, please list your information below. Your spouse’s membership extends to you as well, and providing your information ensures that we can provide you with assistance and keep you informed of changes to military benefits. We also have information available in the Survivor section of our website.


We will send an email to confirm receipt of your report and we encourage you to contact us if there is any way that we can help the family of the member. We are available at 1-800-234-6622, Monday through Friday, 9 a.m. to 5 p.m. Eastern time.

For privacy of family members, entries to the Sounding Taps column must be requested by a family member or executor of estate. Those individuals can request the listing by using this form, emailing taps@moaa.org, or calling the Member Service Center at (800)234-6622.
